[BSOD] [Crashes] - Months long BSOD issues.

So, my current predicament.

Since about two months ago I have been having issues with some apps and games that previously ran normally inexplicably crashing the entire desktop via a BSOD.
When this first started, it was largely spewing out errors about OpenGL driver failures. After some time with the PCS tech support and a variety of small fix attempts they decided to RMA the GPU to see if this fixes it. It removed the error logs about OpenGL driver failures but the BSODs still occur.
I am putting this out here to see if anyone might have any idea of where to go from here.

The BSOD Process:
The BSOD occurs usually when running games or programs.​

Usually I do not get to see the actual BSOD screen. Instead, while running the program, it will eventually abruptly and completely freeze. The sound will continue in the background and loop (though on one occasion I had a Youtube video in the background keep playing until everything fully died).​
After this, eventually, the computer will simply shut itself off and restart itself leaving me with logs about a BSOD in the event viewer.​

Inconsistency:
One curious thing I have noticed about the BSODs is that they are also inconsistent between the programs.​
For example, some games that cause crashes will be far less hardware demanding and will crash almost instantly, while others which are intensely hardware demanding might not crash at all. For some, I was able to run them without crashes until some specific action, like opening a menu at which point this was always a guaranteed BSOD, but anything else without doing this action was perfectly fine and caused zero issues.​
Ultimately there doesn't seem to be some logical pattern that I can pinpoint. Some apps will simply BSOD it within minutes of runtime while others do not or do but only in specific circumstances. The actual hardware intensity of the app doesn't seem to correlate to this.​

Some of the things that have been tried already:
  • Replacing the GPU after the RMA + reseating it several times yields no results
  • Repeated driver uninstalls for both IGPU and dedicated GPU via DDU followed by fresh reinstalls, tested both via file and via software. No results, issue survives this
  • Windows updates / health commands do nothing to it either
  • At the start of the issue, did some CPU tests + mem86 for about 3 hours at request of PCS tech support. Both results showed nothing.
  • Several clean windows reinstalls from USB did nothing to stop the issue either.
One thing that does seem to make a slight difference:
  • While trying out different things, I noticed that if I use MSI Afterburner to reduce the GPU clock speed and limit the power a little, it seems to on some occasions delay the inevitable BSOD. Trying to lower the clock speed and power limit even more has diminishing returns. Lowering it by itself seems to delay it but the amount of lowering doesn't seem to have an impact on fully preventing it or making it delay longer.

Both of those errors relate (I suspect) to the same TrackIR device (USB\VID_131D&PID_0159). Since that seems to also have graphics implications, and your BSODs are graphics related, I think that's where you need to start. Either remove and uninstall that TrackIR device as a test and see whether the BSODs stop, or get those drivers installed and up to date.

As mentioned, the dumps are all similar, they all fail in a graphics operation, so we are looking at the 4090 or its driver. You said that you'd reinstalled both the card and the Nvidia driver, and you do have the latest driver version (566.14), so they are probably OK.

I would start with that TrackIR device.

Appreciate the response!

I have now both disconnected and uninstalled the TIR software from the system, to my knowledge completely.

Had another bluescreen despite of this. This time accompanied by several application events from NVIDIA OpenGL Driver simply stating "ran out of memory"

Can you run the Sysnative data collector app again please and upload the new output?

Also, enter the command perfmon /rel into the Run command box. The Reliability Monitor will open, please post a screenshot of that display.
